At Adult Medicine & Senior Health Services, we focus oal: to At Internal Medicine & Senior Health, we focus on one goal: to address the needs of the older patients who use the facilities of West Florida Hospital. The system includes physicians encompassing virtually all medical specialties, support staff, and a variety of specialty services which are recognized across the nation. With professionals who know the patients, their families, their medications, and their needs and desires, Internal Medicine & Senior Health is designed to coordinate that care.
Coordinated care is the key to Internal Medicine & Senior Health. By training, by interest, and now by location as well, the staff is a team with a mission: improving the quality of life for seniors. To do that we have brought together full-time, board certified geriatricians, advanced registered nurse practitioners (ARNPs), and a nurse care manager with experience in working with seniors. All of our staff are here because they especially want to serve older patients and their families.
At Internal Medicine & Senior Health, seniors and their families will find a team whose focus is on seniors, a team who becomes familiar with the total picture of each patient's health and coordinates the care to make sure all needs are met in the best way.
Internal Medicine & Senior Health is conveniently located at 8333 North Davis Highway in the West Florida Medical Tower building.
Services/Programs
Primary Care
Board-certified geriatricians and advanced registered nurse practitioners (ARNPs) with special training and experience in geriatrics deliver primary care services. Care is provided across the continuum in the office, hospital, nursing home, ALF and retirement communities. Referrals to specialists will be coordinated to assure good communication and continuity of care.
Case Management
An RN Case Manager will assist high-risk patients in arranging necessary support services to meet health care needs. Each patient's care will be monitored to maintain optimum function and well being.
Consultive Geriatric Care
On physician referral, the geriatricians and other members of the team will provide a thorough assessment and develop recommendations for a plan of care specifically designed for the patient and his or her primary physician.
Education
Key to health maintenance, education is a priority with Internal Medicine & Senior Health. In addition to one-on-one education, community seminars are offered at both locations and through presentations for organizations throughout the community. Professional geriatric education is a priority, too. We are a leader in the medical community, coordinating continuing medical education conferences at West Florida Hospital and sponsoring an annual gerontology conference for professionals.
